You can use any of the survey tools out there, like Google Forms, SurveyMonkey, or Typeform, which will all aggregate the responses in a single spreadsheet. Then, have your survey partner in crime synthesize the responses, delivering it to you verbally and lovingly first. Then have her follow up with the raw answers.
When I got my answers, I was overwhelmed, emotional, and unsure of how to digest their confidence in me:
